Workers' Compensation Endorsements

Please E-mail all endorsement requests to WCEndorsements@ArrowheadGrp.com. Our goal is to complete 90 percent of endorsement transactions within 10 business days; increased volume during peak quarters may effect this processing time. Please keep this timeframe in mind when checking the Daily Print for expected documents.

Zurich Blanket Waivers:

Requests for blanket waivers should be made at the time the bind order is submitted. Blanket waiver requests received after inception will be processed mid-term.

 

In order to consider a blanket waiver, the following information is required:

1. A list of jobs from the past six months and any prospective jobs (Please include a brief description of the duties the employees are performing)
2. A description of the safety procedures for employees at the worksite or construction site

 

ALL OTHER CARRIERS: Blanket Waivers:

Requests for blanket waivers should be made at the time the bind order is submitted. Blanket waiver requests received after inception will be processed mid-term.

 

 

Specific Waivers:

In order to consider a specific waiver, the Job Exposure Worksheet must be completed in its entirety.

 

Important Reminder:

Specific and blanket waivers do not rollover to a policy that has renewed. You will need to provide these specific or blanket waiver requests to your underwriter at binding so they can be processed for each renewal.
