To Obtain a Quote

• Completed ACORD Applications 125, 126S (140 or 146 if Property/IM coverage desired)
• Completed Supplemental Application
• Your rate indication from Arrowhead Exchange

To Bind

• Signed ACORD Application
• Signed Supplemental Application
• Three years currently valued loss runs or a three-year loss statement signed by insured.
• Completed Surplus Lines paperwork
• Your request to bind coverage stating the desired effective date and the payment plan requested.
• If outside finance is requested, provide name and address of finance company or copy of agreement
