California accounts below $20,000 in manual premium must be submitted online. Our system will confirm eligibility and generate the required submission documents. No documents or underwriting will be required until the time of binding.

Submissions above $20,000 in manual premium must include the supplemental application, 3-4 years of currently valued loss runs and the updated Acord 130. Multi-state accounts must be submitted via email to WCSubmissions@ArrowheadGrp.com.

Our goal is to provide the highest quality service to our producers and policyholders. With extended hours, our customer service team can provide status of audits, billing transactions, cancellations/reinstatements, endorsements, loss run requests, policy issuance, and submissions.

We also aim to complete 90 percent of endorsement transactions within 10 business days. Increased volume during peak quarters may affect this processing time. Requests can be sent to WCEndorsements@ArrowheadGrp.com.

Claims Handling

Each of our workers compensation carriers use the expert and efficient claims handling through American Claims Management, Inc. (ACM).  ACM is a carrier-minded claims administrator who works closely with our underwriting department.

Our goals are aligned to reduce claims costs and improve the profitability of our program. ACM provides superior outcomes as a direct result of their professional team, advanced technology and pricing integrity. With ACM as your claims administrator you will receive the following competitive advantages:

  • A dedicated claims adjuster who will contact our mutual client within 24 hours of bind confirmation.
    Introduction from adjuster will include a review of our claims kit, claims reporting options, and medical provider network.
  • Quarterly or semi-annual claims reviews and advanced reporting services (upon request).
  • Dedicated in-house team focused on medical cost containment, aggressive fraud investigation, and recoveries.
  • Experienced and bilingual adjusters have a minimum average of 7 years of claims experience.
    Management team has an average of 22 years of claims experience.
  • Lowered costs with integrated technology.
  • Real-time loss runs and Plan of Action reports.
    Monthly, quarterly or semi-annual report subscriptions are available (upon request).

Workers’ Compensation

Final Audits

All Workers’ Compensation policies are subject to a final audit. Arrowhead and all its audit vendors conduct voluntary, telephone and physical audits depending on the written premium and carrier.

Audit Worksheets can only be sent to the insured. To dispute a final audit, the policyholder must submit a formal letter on a company letterhead, signed by an officer, along with supporting payroll records. Indicate specifically what is being disputed and provide supporting records.

Payroll

Provide supporting payroll records and/or documentation.

Dual Wages

Supply time cards and corresponding paystubs for the employee(s) in question.
